Removing Barriers to Participation in the Virginia Cooperative Extension Diabetes Prevention Program through the action of Community Health Workers: Process and Preliminary Outcomes

The Virginia Cooperative Extension (VCE), in collaboration with the Virginia Department of Health is pilot testing a model for employing Certified Community Health Workers to increase physician referrals, enrollment, and retention of high risk populations into the VCE Diabetes Prevention Program. Participation in public events, direct outreach to physician practices, direct referrals from Medicaid insurers, and one-on-one follow-up by the CHWs to address social determinants of health barriers are strategies being employed. The model process and outcomes of the four strategies will be presented during the session.
